Remove most dialyzer warnings

This is a large change set, which adds type specs
needed for dialyzer to be happy. We likely
need even more type specs as we move forward.

Improve the test stale btree removal.
...
Also delete the "simple" Btree in between test runs so we have a clean
slate of what is happening.
